Relevance DOES matter when getting links to your site. Links from some social bookmarking site is not going to get me any 'points' with Google because the incoming link from that site is in no way relevant to a medical practice, which is what I primarily build. That's why link farms are such a BAD idea - the incoming links are not relevant.
