Can anyone recommend some online survey software. A site visitor should be able to
- click check boxes for multi-choice answers
-enter comment into free type fields

The software should collate results and possibly provide tables, data for export as .txt, .csv or .xls

Any ideas?

well...you can either code up a form in PHP yourself.

you can add a CGI yourself and do it in HTML.

or you can get a website service to do it...

www.response-o-matic.com does a great job in that. and its highly costumizable in HTML

You should use html to make the survey, and php to put it back into a result html form. Its simple once you learn a little PHP. Took me half a day to understand how to use php. Look up some php tutorials, more specifically form tutorials online. Good luck

Obviously, if you program a script yourself, the program most likely won't be very complex (unless you're willing to spend weeks and even months developing it). Remotely hosted solutions are also very limiting, and they usually contain ads. I think the best choice is to find a script here. Some of them are simply polls though and may not be what you want.
