I am going to start my online advertising campaign to increase my sales

and

i need visitors to come and start submitting their information on the form.

I dont exactly know how to design the page?

How to convince the visitor?

How much input fields will be sufficient?

I think what you need is a "landing page" -

some key headings, testimonials, case studies and of course, a form to harvest data.

You'll need to have a good understanding of marketing otherwise seek some advice from a marketing person and then get the notes and pass it onto a graphic designer who can then spice it up and load it on the net for you.

when i saw it ,i think that's a professional website.although the content is limited ,it is a new page,right?the color and css is normal but attractive,in fact,clear is a improtant factor ,you use yellow to match it,it is smart ,that let me feel warm.i didn't read the words clearly,if it is useful for the guy who visit ,that's perfect.this page is successful.
