Nice review. Unfortuntly it wont stop those that buy unmannaged vps and have no clue what there doing and try and start a host.

I had safe_mode on for over a year but it started to get annoying with so many people having problem with blocked functions so I disabled it and now I just have a huge disable_function list

Safe mode on/off - it is an interesting question. For security reasons it is always better to set up the safe mode off. But many free PHP scripts which clients download from sites like Host Scripts require safe mode on.

Even if you apply all security measures you can not be 100% safe. It is always best to order server hardening once in a while.

you also need to secure/log your SMTP server (I suggest a different server for SMTP). if someone is abusing your SMTP server, you will know who and can take proper actions. someone can easily make a PHP script to send anonymous emails from your web server and can get you in deep trouble (happened to my friend's hosting website. he was warned by his ISP).
