I think page title is the most important followed by internal link structure and then meta tags. These are the basics which I apply.

For on page optimization first you should have unique and fresh content with anchor text linking back to other pages of the site. Then keywords and keyphrases. They should be strong and less competitive. Now you've to look after your page title, URL, H1 tag, ALT tags for images, site map etc.
