As for the "code" it is clean enough and is just a simple form that is processed at webconf. What I don't see is what webconf does with the input.

I'll have to check out what webconf is actually doing to determine if there is any harm, or help, in placing the form on a page or not.

Ok, a bit more information.

From the webconf site:

Unless you have a page featuring SEO tools, adding this form is not going to help you at all. Also, I did use the one on webconf and saw that it did return sites with similar themes that accepted links (paid or not paid) but would have had to wade through each one since some of them were actually simple link directories and not sites with "real content" related to my topics.

I would not put that tool / form on my own site.

I'm not one for inline styles, your code looks too cluttered and messy. You could quite easily separate your presentation from your content if you put your styles in a stylesheet.
