Contact form w/multiple recipient option?

I am looking for a simple PHP contact form but would like to be able to have the person select from a list of recipients.

example: they could select sales, support, or other.

I am doing a website for someone that has 3 or 4 departments. They would like to have 1 contact form and just have the user select who will get the email...

All help is greatly appreciated!

I could do this for you pretty easily. I have a contact form script that I have made in the past, I will try to find it and edit it to your needs. I will post back here or send you a PM. Also, feel free to PM me.
