I want to learn PHP. I had started a while ago but stopped for a while and now find myself lost. I need a simple email form.

The form would ask for the following info:

FULL NAME
AGE
WEIGHT
CITY/STATE/ZIP
FIGHTING STYLE
YEARS TRAINED
FIGHT RECORD
TITLES HELD
PRO or AM?
SCHOOL/AFFILIATION
Myspace Address

and then an option to attach a photo... (not required though)

I would love help making this, then I would disect it and use it as a learning tool.

The form would just email the results.

This is for registering for a mixed martial arts event
