What are the basis to choose the right keyword for every page of sites..?
what should i do so that I will be the top in SERPs of my target keywords..?

Try Google Adword keyword tool,wordtracker,overture keyword tools

Sometimes it's a crap shoot...you think you found a winner, try a few others you think are good and one of the one's you least expected to perform kicks butt...

It is better to choose a keyword phrase, do not go for single or general keyword. Then avoid keywords that have huge competition, its always better to go for keyords that have less competition with higher search volume. You can use google suggest, wordtracker or adwords tool to select your keywords.
