Most forums have an option that requires admin to approve memberships before people are allowed to login.. You aren't being very specific.

i think what he means is a php form which gets emailed to him try looking on hotscripts and it will use php not html

PHPBB has an option where the administrator has to approve members. You can also turn registration off and just require people to e-mail you and sign them up yourself.
