Hello, I would like to make an email form for a friend's website.

They want text boxes and for it to say next to these boxes, things like "First Name", "Last Name", "Address", "City", "Postal/zip code", "Telephone", "Fax", "Email", "Comments", and "How did you hear about us?"

HELP, I havent a clue how to do this and I would like for someone to give me step by step directions, please?

I forgot to add that at the bottom of all this they want "Send" and "Clear Form" buttons.

You may have just made my day, 'cause I have tried about 3 different Mail To scripts which have been completely bogus off of Google: though Free, nontheless.

You are going to want to inclue CAPTCHA image for security reasons. Otherwise you will get spammed to death.

i assume your friend wants the email form for an opt in offer. If he is using one of the more reputable autoresponder services like aweber or getresponse they provide a form generator with the service

they have a nice free form maker at jotform.com

I use it for one of my sites that requires customer inquires. They give you easy cut and paste iframe code.

If you are using WordPress as a CMS for your website, I strongly recommend the cForms II plugin. It makes generating contact forms a breeze and is very powerful. You can get a lot out of it, and can track the emails in your WP backend, if you so desire.

I'm using cForms II for my websites' contact forms and it is great. Without a doubt the greatest thing since sliced bread. Get it here.
