I have just registered and have a question reguarding submission forms. I am currently trying to add a page to my site that allows somebody to fill in a form. I can then obtain this information.

I would like a submission form that works like this example:

Hopefully someone can advise me how I would go about this.

What exactly you want it to do??

Its just a standard form..

If you mean because the form fields change color when clicked thats done with CSS... if you need help with that just ask
