I am starting a site where people can send in ideas, and then I will add them.

I need a form field set-up like this:

Application name [ single row ]
Application description [ 4 row text area of the same width as row above ]
Browse for image button [ file path row ] Add another image (link)

Submit button

Ideally, the words Application name and Application description would be included in light gray in the text field and text area.

Add another image would open a new Browse for image button below.

Submit submits the entire idea to my e-mail address.

this requires some sort of more advanced script such as php. ask in the php section and someone will help (i could probably do it, but have other things to do)
