Help with a Dreamweaver form

I copied this process.php from a website but cannot get it to work as it comes up with parse error on line 12. Can someone help please.

You seem to have parts missing?

You need the html form part and then the php SEPERATE page...

try going here to have the form generated for you, then you can see how things should be.

You don't need a seperate php page if the form targets the same page it is on.

the end of the mail () function needs to be closed with a ); instead of >

,"$headers");
not:
,"$headers">

you should be asking this question in the php forum

I may be missing something here as this is not the way I'm used to doing it , but where is the form??
