If I'm not late I can advise you to use Flash To Video Encoder PRO!!! It's the best tool to convert flv to mp4!!!!

Let me add one more tip, as the app I use hasn't been mentioned (:
Flash to Video Encoder by GeoVid does the job fine too. It converts not only to mp4, but practically to any popular video format.

try ojosoft's flv to mp4 converter, I found that one worked the best for me so far, excellent quality and no breakup of the files.
