Affiliates, zipcode and email ads?

I need some advice on affiliates. I'm new to affiliates. I see ads that require zipcode and some that require email addresses.

Now my question is, do I have to build the form to send this informaiton to them or do I just put the ad on my website?

Usually they already have landing pages build and ads built... you just have to bring traffic to it

Yep. All you have to do is find a way to drive traffic (would be best if they were actually interested in something like that) to the site and watch your stats.
