Azoogle W9 Tax form

I am in the process of signing up for Azoogleads, and they requested that I send them "a photo copy of your original W9 application form or equivalent piece of ID matching your Tax Identification Number with your name/address"

I think they are asking for this, correct? (same form as I filled out for AdSense):

I just wasn't certain due to the wording and wanted to make sure

I'll call my rep there to ask once I get home, but figured you guys probably already know and can tell me.
