i think you can get w9 form from irs.gov. Its a real simple tax form to fill out . Then you just have to fax it back to them. I think they have to get it mailed in before they pay out any money
I have been waiting for weeks. A rep contacted me asking me for my site info, I replied, wrote them another email asking for the status, got a standard "We're going through applications" email, and now I'm still waiting.
They are extremely successful, extremely busy, are very thorough checking affiliate apps due to affiliate fraud and great at support. So they are worth the wait to get approved.

Azoogle has some really good offers and if your into rewards they have laptops and stuff you can get if you hit a certain threshold per month. But then again if you make 100k a month you can probably afford to go out and buy your own 32" Flatscreen HDTV or Laptop..lmao
there rewards are nice to get though even if you could afford to go out and buy them getting something ontop of what you are already making is nice

You won't be giving your SS# to a random site. Azoogle is 100% for real and they are nothing but a legitimate business. They will send you a link to a W9 form, you print it out, fill it out, and fax it back. A real person gives you a call before any of this happens. Then your account manager will call you. It's legit, no need to worry.
It took me a little over 24 hours to be approved. I used Chickenhole as the referral and I called Azoogle to speak with my soon to be account manager. She sped up the process.
I get about 8,000 uniques with my sites total. That isn't much, but that combined with me having 6-7 sites was probably enough.
The campaigns that work the best for me are the ones where they enter emails or zip codes. My personal best is the $1000 free gas card.
