Very.
Directory installations (i.e. site.com/blogname) don't work. So only the vhost (blogname.site.com) installs work and are supported (barely).
If you don't know much about apache, you may have difficulty getting it to work. Your host needs to be willing to setup a wildcard dns entry, and a wildcard apache vhost entry.
If mod_rewrite isn't setup just perfect (to WPMU's liking - and no one can quite figure out exactly what WPMU actually likes), it won't work.
With lots of users, the database gets huge.
PLUS...spam blogs...lots of them!
It is a good idea, and eventually, I think WPMU will be just fine...but just not now...which is why it isn't even in Beta yet.
