Ok, I am assuming that the name, etc. are a variable created.
You should really try and restructure the whole page and do away with the response.redirects. Make the page direct back to itself and catch all errors and if successful let the main page give the success or thanks.
Also check and make sure that CDONTS is installed properly and all permissions are set on the mail folders on the serverside.
