moastly, when a image is very poor i use a process which takes A LOD OF time. First of all use the pen tool and the convert point tool to make a selection of great one-color sections. After that try to make the rest of the image with brushes, blur, smudge dodge and burn. Be sure that you duplicate the 'original poor image' on a layer so that you can go back is that is needed. Maybe you shoudn't retouch everything but most of the retouching is just a lod of patience, experience and maybe even a bit of luck.
